You are eligible for membership in our plan as long as:

	•	You live in our geographic service area
	•	-- and -- you are entitled to Medicare Part A
	•	-- and -- you are enrolled in Medicare Part B
	•	-- and -- you do not have End Stage Renal Disease (ESRD), with limited exceptions, such as if you develop ESRD when you are already a member of a plan that we offer, or you were a member of a different plan that was terminated.

Medicare beneficiaries may enroll in Leon Medical Centers Health Plans through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.

1.	Click on link below. Please note that upon clicking the link below, you will be leaving our website and continuing on to Medicare's own official site. http://www.medicare.gov.
2.	Click on “Health and Drug Plans” on the Medicare website.
3.	Then click on “Compare Drug and Health Plans”. On the right of the screen, under Additional Tools, click on “Enroll Online” button.
4.	Enter your Zip Code and Enter Plan Name: Leon Cares Then click on “Enroll in a Plan.” You will see some of the basic benefits offered by Leon Cares.
5.	Click on “Enroll Now” button on your right. On this page you will see a complete list of benefits offered by Leon Cares.
6.	Click on “Continue to Enrollment” button located on the top right of the page and also on the bottom right of the page.
7.	Click “Continue Enrollment” button on right where you will “START ENROLLMENT.” Provide your personal information by filling out the form. Please have your Medicare card or information ready. Complete the online application, then click “Continue” on the bottom of the page. After you have finished completing the application, click on the “Submit” button.

You have successfully submitted your online application to become a member of Leon Medical Centers Health Plans.

Leon Medical Centers Health Plans will be contacting you within 48 hours to confirm your enrollment.
