When a Member enrolls in Leon Medical Centers Health Plans the Member will receive an ID card. Members should carry the card all of the time. The ID card serves as immediate proof of membership in Leon Medical Centers Health Plans. A Member needs to show an ID card whenever receiving medical services covered by Leon Medical Centers Health Plans such as:

		Doctor’s office visits				Emergency room treatment
		Hospitalization				Laboratory and x-ray services

Your ID card will contain the following information:

	1.	Your Name — You will receive a Leon Medical Centers Health Plans. ID card to be used whenever you receive health services from your PCP and other health care Participating Providers.
	2.	Identification Number — You will have your own Leon Medical Centers Health Plans identification number.
	3.	PCP Name and Telephone Number — Your Primary Care Physician’s name, address and telephone number.
	4.	The Telephone Number of the LMC Health Plans Member Services Department. You can reach the plan at this number from 8:00 am to 8:00 pm eastern time, 7 days a week.
	5.	Start Date — The date that you became eligible for coverage through Leon Medical Centers Health Plans. If your coverage is terminated for any reason, your card is no longer valid. Leon Medical Centers Health Plans. will not cover any services received past the termination date.

During the time you are a member of our plan, you must not use your red, white and blue Medicare card because it is used only when you are getting your Medicare through Original Medicare, and our plan is different from Original Medicare.

So we tell all our new members, keep your red, white and blue Medicare card in a safe place because you might need it later on if you return to Original Medicare. But during the time you are a member of Leon Medical Centers Health Plan, be sure to use only your Leon Medical Centers Health Plans member ID card. Otherwise, your care might not be covered and you’ll have to pay for it yourself.
